In the first episode of a two-part story, Burke orders a "Code Black" when a WW2 re-enactor is brought in with a homemade bomb embedded in his chest. There's tension aplenty as the man is brought into surgery while young paramedic Hanna Davies nervously holds the explosive in place and bomb-squad detective Dylan Young tries to maintain order and calm. Elsewhere, Alex and Izzie undergo life-altering experiences that neither could ever have dreamed of; and Bailey finally goes into labor while her husband is nowhere to be found.

GREY'S ANATOMY OPENING QUOTE:
It's a look patients get in their eyes. There's a scent, a smell of death. Some kind of sixth sense. When the great beyond is headed for you, you feel it coming. Whats the one thing you've always dreamed of doing before you die?
OK , Hello, Clearly not my dream!
Izzie: You're the strongest most masculine man I have ever known.
Cristina: You're smarter than me and you've got really great hair.
Meredith: I'm in love with you, George. I always have been, and I always will be. Meredith [voice-over]: [George falls out of his bed] See, I told you, not my dream

GREY'S ANATOMY EPISODE TRIVIA
Goofs, behind the scenes facts, and little-known tidbits...
- After Meredith puts her hand on the bomb, and everyone stands up. It goes to a shot of Burke, and he is standing straight up. But when it goes to a shot from up above, it shows him kneeling down.
- Near the end of the episode, Mrs Carlson is with the Chief in his office, and the Chief asks Alex to get her some coffee. However, at this time Alex should be busy with Izzie in the on-call room. We can tell this because before the coffee call, Alex and Izzie are in the on-call room, and it is not until after the coffee part that Alex and Izzie come back fixing up their clothes and hair.
- When Alex runs out to warn Burke as he is running through the hall there is nothing on his head but when he gets in he is wearing a scrub cap.
- Jillian Armenante who played Mindy Carlson later played Arlene in Private Practice.
